When should you start pelvic floor PT during pregnancy?
Most women first hear about pelvic floor physical therapy after something has already gone wrong. But some of the best outcomes come from starting during pregnancy — sometimes well before any symptoms show up at all.
The short answer
If you're feeling good, the early second trimester is a great time to start. If you're already dealing with back pain, hip or pubic bone pain, or leaking, there's no reason to wait — earlier is better. And if you're approaching your due date and want to prepare for labor and delivery, even the third trimester is valuable.
Why earlier helps
Pregnancy changes your body quickly. Hormones loosen your ligaments, your center of gravity shifts forward, and your growing uterus changes how your core and pelvic floor work together. Starting pelvic floor PT early means you can get ahead of the common aches — rather than chasing them after they've set in — and build habits and strength that carry you through all three trimesters.
One patient who came to us as an aerialist was able to keep up with her classes until 27 weeks with the right coaching. Another, dealing with significant pelvic pain in her third trimester, walked away with tools she was still using three years later. The earlier the start, the more we can do.
What pregnancy-stage PT actually involves
It's not bed rest and it's not "take it easy." Depending on where you are, a typical plan combines hands-on manual therapy for pain relief, safe movement and strength coaching so you can keep exercising with confidence, and — as you get closer — birth preparation: pushing positions, perineal massage guidance, and breathing strategies for labor.
Is it safe?
Yes. Pelvic floor PT during pregnancy is widely used and, with a specialist guiding it, adapts to your body as it changes week to week. We regularly help pregnant patients keep doing the activities they love, safely, right up until delivery.
The bottom line
You don't have to wait for a problem. Whether you're early in pregnancy and want to prepare well, or further along and already uncomfortable, pelvic floor PT can make pregnancy more comfortable and recovery faster.
